2012 SUMMER RECREATION PROGRAM
JULY 3-26 2012 Tuesday and Thursday 9am until 12 noon West Hurley Park, Dug Hill Road, West Hurley NY Optional field trips on Wednesdays
In order to register your child you will need one of each form per child: -Registration Form -Acknowledgement of Risk Form If your child will be taking the bus we will need one bus permission form per child. The transportation fee is $40 for the first child and $30 for each additional child (we need a minimum of 30)
Registrations after June 29, 2012 will be subject to a $25 administrative fee.
